Derbyshire Independent Living Service extended
We're delighted to announce Revival will continue to support Derbyshire residents through their independent living service, which has been extended for another 12 months.
Revival work across the county to support over 55s, helping them live safely in their own homes. The team provide a 24/7 falls recovery helpline as well as financial support, fall alarm applications and help at home following a hospital stay. They also connect customers to their community to help them feel less isolated at home.
The service has been a lifeline to 485 customers in the last year. Revival team leader Emily shares how customer Jayne* would 'be lost without the service'.
“80-year-old Jayne was living at home with her husband" explains Emily. "She was referred to Revival after falling at home multiple times. We discovered she had several health conditions that were affecting both her physical and mental health. We first installed a community alarm in case of future falls and then worked with Jayne to create a support plan to help keep her safe and well at home. The support has helped Jayne and her husband 'feel safer' and has hopefully prevented any future falls and calls to the ambulance service. They both said they’d be 'lost without our support'.”
Emily said she’s thrilled to have the service extended for another year.
“We’re over the moon to have our service extended,” says Emily. “We're supporting around 135 customers a month, as well as working alongside statutory and voluntary agencies to provide vital support in our communities.”
The service will also continue to play a vital part in reducing the pressures faced by NHS services.
“Derbyshire is a big county" explains Emily. "There are some parts that are quite remote and hard to reach. If our service didn't exist, these people would have to rely on 999 and in the event on a fall, could be on the floor for a long time which could then cause additional health problems. Instead, our customers are seen within 45 minutes, helped up off the floor, given support and if needed, connected with their GP”
