Revival’s hospital discharge service is celebrating the impact it has made supporting vulnerable people across North Staffordshire and surrounding areas.
As the service draws to an end this June, we’re celebrating the incredible impact this partnership work has had for local people leaving hospital.
Working from the discharge hub at Royal Stoke University Hospital, the service has supported more than 1,200 customers over the past two years, helping people leave hospital safely and remain independent at home.
The service has worked alongside health, social care and voluntary sector partners to remove barriers preventing safe discharge, reduce delayed discharges and help prevent avoidable readmissions.
Support has included practical interventions such as food parcels, furniture provision, property clean-ups, medication support, white goods, heaters and wellbeing support.
The service has also played an important role in strengthening partnership working across the VCSE sector, improving referral pathways and ensuring customers can access ongoing support within their communities.
Many customers supported by the service were experiencing vulnerabilities linked to poor health, housing issues, isolation, disability, poverty or self-neglect.
Through coordinated, person-centred support, the team has helped customers transition safely from hospital back into the community while reducing pressure on wider health and care services.
One customer supported by the service was discharged safely after staff coordinated medication support, mobility support and emotional wellbeing interventions following a hospital admission after a fall.
Another customer was able to return home sooner after the team sourced and arranged delivery of specialist furniture to meet her temporary mobility needs.
The service has also supported professionals to make safer discharge decisions by identifying risks within home environments and coordinating alternative support where needed.
Operations Manager Katie said: “Hospital discharge can be one of the most vulnerable and uncertain times for people. We are incredibly proud of the impact this service has had supporting customers to return home safely, maintain independence and access the support they need within their communities.
“This work has only been possible because of the dedication of staff and the strength of partnership working across health, social care and the VCSE sector.”
