For the past five years, Destination:Home has been changing lives and shaping new chapters for local people impacted by homelessness in Stoke-on-Trent.
Led by our homelessness specialist, Concrete, and powered by a partnership of passionate local organisations, this pioneering service has stood alongside thousands of people experiencing homelessness - offering not just a place to stay, but the tools, support and belief to truly thrive.
Through 24/7, tailored support, we’ve helped people to:
- Find safe, secure homes
- Build confidence and independence with tenancy and life skills support
- Access mental health and wellbeing services
- Gain training, employment and new opportunities
- Connect with peer mentors who understand their journey
At its heart, Destination:Home has always been about more than accommodation. It’s about hope, recovery and lasting change. Together, we’ve proven what’s possible when organisations come together with one mission: to make homelessness history.
As we look to the future, we celebrate the incredible impact so far - and recommit to walking alongside those who need us most, for as long as it takes.
