For the past five years, Destination:Home has been changing lives and shaping new chapters for people affected by homelessness in Stoke-on-Trent.
Led by our homelessness specialists at Concrete, and powered by an incredible partnership of local organisations, this pioneering service has stood shoulder to shoulder with thousands of people - offering not just a place to stay, but the belief, tools and support to rebuild from the ground up.
Through round-the-clock, person-centred care, we’ve helped people to:
🏡 Find safe, secure homes
💪 Build confidence and independence through life skills and tenancy support
🧠 Access mental health and wellbeing services
🚀 Take steps into new opportunities such as training and volunteering
💬 Connect with peer mentors who truly understand their journey
At its core, Destination:Home is about far more than accommodation. It’s about hope, recovery and lasting change - proving what’s possible when compassion and collaboration come together with one shared mission: to make homelessness history.
As we look to the future and prepare for the next chapter, we’re celebrating the extraordinary impact this partnership has made - and we remain committed to walking beside those who need us most, for as long as it takes. 🐝
Referrals
We received 1,500 referrals to our Destination:Home service last year.
LGBTQ+ customers
Last year we supported 70 customers through Phoenix, Destination:Home's LGBTQ+ service.
Floating support
Destination:Home provided floating support to help 62 local people stay in their homes and avoid homelessness in 24/25.
Lived experience placements
Destination:Home carried out 7 placements for local people with lived experience, supporting them to find a new chapter and ensure that lived insight develops our services.
Celebrating the impact of Destination:Home

Tim's story
After a family breakdown left him homeless for a year, Tim* found support through Destination:Home. Struggling with severe anxiety and depression, he initially kept to himself, but with the help of his support worker he built confidence to get outside, manage his health and take control of his wellbeing. Now settled in his own home for six years, Tim is proud of how far he’s come - and so is his family.

Ellie's story
After escaping an abusive relationship and time spent living on the streets, Ellie* moved into Destination:Home's 24/7 female scheme. At first, the idea of managing a tenancy felt overwhelming, but with support from staff she’s grown in confidence and resilience. Six months on, Ellie feels safe, proud of her progress, and excited to take the next steps towards an independent future.

Jack's story
After years of addiction, health problems and homelessness, Jack* came to Destination:Home feeling isolated and ashamed. With dedicated support, he’s detoxed, received vital medical care, and reconnected with his daughter. Now three months sober and in stable accommodation, Jack is focused on his recovery and building a more independent future.
