Introducing the next chapter of Destination: Home in Stoke-on-Trent.

For the past five years, Destination: Home has supported thousands of people across Stoke-on-Trent to move away from homelessness and towards safer, more stable futures.

We’re delighted that Stoke-on-Trent City Council has commissioned the extension and expansion of this vital service, enabling us to build on its strong foundations and support even more people, in more ways, across the city.

Led by Honeycomb Group through their homelessness specialists at Concrete, the expanded partnership brings together Adullam, North Staffs Mind and brand-new partner Brighter Futures. Together, we are strengthening our collective expertise and capacity to deliver a more integrated, ambitious and preventative service for Stoke-on-Trent.

Outreach support

Reaching people early, building trust and reducing harm - particularly for those sleeping rough or disengaged from services.

Emergency accommodation

Providing immediate safety and support when people have nowhere else to turn, alongside planning for next steps.

Accommodation-based support for people experiencing multiple disadvantage

Specialist, trauma-informed support for people facing complex and overlapping challenges such as mental ill health, substance misuse and trauma.

Floating homelessness prevention

Flexible support for people living in the community who are at risk of losing their home - helping prevent crisis before it happens.

Move-on support

Supporting people to transition into more independent living, sustain tenancies and build confidence, skills and connection.

    Concrete

    Concrete is our specialist homelessness service. Our team support local people across Staffordshire and Cheshire East, helping them find a safe place to call home and start a new chapter.

    Brighter Futures

    Since 1974, Brighter Futures has provided accommodation-based support in Stoke-on-Trent for individuals with complex needs, specialising in  homelessness, vulnerable women, and learning disabilities

    Adullam

    As a specialist provider of housing and support services since 1972, Adullam works in partnership with local authorities and other public sector partners to house and support thousands of people across England and Wales.

    North Staffs Mind is our local mental health charity, supporting the mental health and wellbeing of people in Staffordshire for almost 50 years.
